Waiver
In consideration of being allowed to enter UrbanSplat Paintball, use the equipment and facilities, and/or participate in any party, function, or program at UrbanSplat Paintball, the undersigned on his/her own behalf and on behalf of the minor child(ren) identified below acknowledges, appreciates, and agrees to the following conditions: 

1.              I represent that I am the parent and/or legal guardian of the child(ren) identified below as participants, or in the alternative, I represent that I have obtained the express permission of the parent(s) and/or legal guardian of the child(ren) identified below as participants to execute this Waiver.
2.             I understand that UrbanSplat Paintball is owned and managed by Bananas! Drop-In Jump & Play, Inc., an Ohio Corporation.
3.             I agree that the participants listed below and I shall comply with the terms set forth in this document as well as the posted “Rules of the Game.”  I agree that the participants shall only use the equipment and attractions only as directed.
4.             I agree that the participants listed below and I shall comply with all posted safety signs and the verbal instructions of the UrbanSplat Paintball staff.
5.             I am aware that there are inherent risks associated with participation in the UrbanSplat Paintball attractions, facilities, equipment, programs, and parties, including the risk of serious personal injury and damage or loss of personal property and equipment.  I, on my behalf and on behalf of the participants listed below, freely, voluntarily, and knowingly assume all such risk, both known and unknown, including, but not limited to, those risks that may arise out of the negligence of other children and adults using the attractions and equipment, the staff managing the attractions or working/repairing/storing my personal property equipment.
6.              I agree to immediately notify the staff of UrbanSplat Paintball of any hazard observed during participation in the use of the attractions and equipment.
7.            I, for myself and the participant(s) listed below, our respective heirs, assigns, and personal representatives, hereby release and hold harmless,  UrbanSplat Paintball., Bananas! Drop-In Jump& Play Inc., its affiliates, officers, members, agents, employees, stockholders, and directors from and against any claims, injuries, liabilities, and/or damages arising out of, or related to, our participation and/or use of the facilities, equipment, attractions, and/or events.
8.             I acknowledge and agree that images digital, photo, and other may be taken of me or any individual listed below. These imagaes are the sole property of UrbanSplat and Bananas Drop-In Jump & Play, Inc. Further, I agree that such images maybe stored, distributed, placed on the internet or used in advertising without my prior consent or any compenasation due whatsoever.
